Our cataloger has been using the New Titles List in the Cataloging/Collection reports to get a monthly count of new books. It seems this pulls from ordering/receiving activity, but we do not always create order records for donated books, so they are not included in that report. Is there another report we could use that would include that data? Alternatively, is it very bad that we don't consistently create donation lists? Like, will that have some other negative effect, or does it just mess with reporting?

The New Titles report uses the Cataloguing universe which is a bit different from other universes, such as the LHR Item Detail universe. You can see a comparison of the universes at the following page: Universes - OCLC Support. The title and holdings information in the Cataloguing universe appears to be updated monthly which is why you might not be seeing some of the titles you are expecting to find when you create your reports. The LHR Item Detail universe is updated nightly.
Regarding your question about creating dontation/gift lists in the Acquisitions module, we tend to create gift lists for tracking and statistical/reporting purposes. If you don't receive that many gifts at your institution, perhaps you might consider creating a gift list that you can reuse for an entire month or longer (quarterly, yearly). You should be able to add gifts to the same list in the Acq module and receive those items/titles over a period of time.

I have several gift lists I use. I've created some for specific donations when there was a large quantity. I've greated one for a faculty member who donates books frequently. I also have a generic one I use for miscellaneous donations and I just keep it as a running list. Since these items are received in Acquisitions, each is assigned an acquired date in the LHR 876 $d subfield.
